Call our office to provide your information and schedule an appointment. After you have set up services and scheduled an appointment, complete the telehealth supplement and ONE of the packets (based on age). Adults and adolescents may bring the completed packet to the first appointment. The parents of children under 14 years must return the completed packet to our office BEFORE the child's first appointment:
